When you come for your appointment, please bring the following items with you:
- Your insurance cards
- Your driver’s license or other photo ID
- Referral form from your primary care physician, if required
- List of current medications with dosage and frequency
- List of current eyedrops
- Current glasses
- Forms listed below (please be sure to complete them before you come):
Payment for services is required at the time of service unless you have an insurance plan with which we participate. All co-payments are due at the time of service. We accept cash, check, Visa or Mastercard.
